The below forms may be used to order new supplies and forms for your AMA-sanctioned event. We also provide AMA logos in electronic formats that may be used to advertise your organization’s or event’s affiliation with the AMA.
Competition Racing Forms
- Competition Event Checklist – Necessary event planning form for AMA organizers
- Supply Order Form – For event items like banners, rulebooks, and responsibility posters
- AMA Membership Sales Report / AMA Membership Sales Report (Tennessee)
- Safety Award and Service Award Order Form – Available to all AMA organizers and is due yearly by October 1st
- Competition Referee Report – A required document for submission post-event
- Competition Injury Report – A required document when an injury at an event occurs
- Adult Event Release / Minor Event Release – Single event releases for both adults and minors
- Competition Protest Form – Logging of protests as outlined by the AMA Racing Rules
- AMA Emergency Response Plan Template – For events that fall under the Alternative Ambulance Requirement Policy or non-speed-oriented events, AMA Competition Rulebook Appendix 6.4
Recreational Riding Forms
- Recreation Event Checklist– Necessary event planning form for AMA organizers

- Recreational Event Entry Paper Form – Optional for event organizers to use to register participants for AMA recreational events
- Emergency Medical Information Form – Optional for road riding event organizers to give to participants to keep in their jacket pockets in case of emergency
- Recreational Post-Event Report – A required document for submission post-event
- Participant List for Recreation Events – For recreational organizers to log participants
- Recreational Riding Injury Report – A required document when an injury at an event occurs
AMA Logos
All organizers are required to use the current AMA logos on promotional materials and web sites. Older logos should not be used.
